An inquiry revealed that a former patient of the same NHS trust that treated Nottingham attacker Valdo Calocane had been involved in a fatal incident less than a year before Calocane’s violent rampage. The incident in August 2022 reportedly included an individual who had prior contact with Nottinghamshire Healthcare NHS Foundation Trust’s mental health services.
The former chairman of the trust acknowledged a missed opportunity to prevent Calocane’s attack, which resulted in the deaths of three individuals. Calocane, who had been under the trust’s care from 2020 until his discharge in 2022, carried out the violent stabbings that claimed the lives of two students and a caretaker in June 2023.
The inquiry highlighted that another former patient of the trust committed a fatal attack in August 2022, but details of the incident were not disclosed. Concerns were raised about the lack of action taken by the trust’s board regarding this prior incident.
The former chairman admitted that the board was informed of the incident in September 2022 but could not conduct an investigation until after the police had completed their inquiry. An action plan related to the fatal attack was only approved in May 2025. Questions were raised about the trust’s handling of discharges and whether proper audits could have prevented the subsequent attacks.
Calocane, who pleaded guilty to manslaughter and attempted murder charges, is currently detained in a secure hospital indefinitely. The inquiry is ongoing to further investigate the circumstances surrounding these tragic events.
